Requirements
The following requirements and reminders apply for the state of Kentucky. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- You must be at least 18 years of age to request for a vital record
- Births that occured from Nov. 22, 1982 - present at the Blanchfield Army Community Hospital on Ft. Campbell Army Base are considered Tennessee births, and certificates must be requested from the Tennessee records office.
- You must be an immediate family member or demonstrate a legal right to the record in order to obtain a copy of the record.
- No physical ID required.
Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 877-684-5852 .
