Requirements
The following requirements and reminders apply for the state of Massachusetts. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- You must be at least 18 years of age to request for a vital record
- You must be an immediate family member or demonstrate a legal right to the record in order to obtain a copy of the record.
- The Massachusetts records office requires that all middle names (if any)are listed on the application
- No ID required unless the parents were not married at the time of the certificate holder's birth, or the record has ever been amended (adoption or legal name change other than marriage) .
- If the parents were not married at the time of birth, or the record has ever been amended, You must fax the requestor's official valid state-issued photo ID.
Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records.

Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 877-684-5852 .
Product Information
The following prices are charged by the state of Massachusetts for the actual certificate and the shipping. The $19.95 fee that is charged by Express Birth Records will be a separate charge from the state's charge.
