Requirements
The following requirements and reminders apply for the state of Minnesota. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- The State of Minnesota requires that you prove your identity by supplying a Notarized Sworn Statement and Photo ID. Print and complete the Sworn Statement and Photo ID, have it signed and stamped by a Notary Public, and then fax it with your order to 888.803.1119 . (the Notary seal must be visible in the faxed copy).
- You must be an immediate family member or demonstrate a legal right to the record in order to obtain a copy of the record.
- IF YOU WERE ADOPTED OR HAD A LEGAL NAME CHANGE (OTHER THAN MARRIAGE), YOU MUST CONTACT OUR CUSTOMER SERVICE OFFICE DIRECTLY TO ORDER A RECORD (1-888-803-1118).
Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 1-888-803-1119.
Product Information
The following prices are charged by the state of Minnesota for the actual certificate and the shipping. The $19.95 fee that is charged by Express Birth Records will be a separate charge from the state's charge.
