Requirements
The following requirements and reminders apply for the state of New Hampshire. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- You must be at least 18 years of age to request for a vital record
- You must be an immediate family member or demonstrate a legal right to the record in order to obtain a copy of the record.

The full mother's maiden names of both parties are required. - You must fax the requestor's official valid state-issued photo ID.
New Hampshire Birth Records will only process requests followed by a faxed copy of the requestor's government issued photo ID.
Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records. - A signature under the copied ID is also required. Unsigned applications will not be processed.

Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 877-684-5852 .
