New York City Birth Certificates ONLY
Requirements
The following requirements and reminders apply for births occcuring in New York City ONLY (5 boroughs): Manhattan, Queens, Staten Island, Kings, Bronx. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- Eligible requestors are self or parent, or demonstrate a legal right in order to obtain a copy of the record.
- Applicant must use own credit card.
- Orders will only be shipped to the correct billing address of the applicant's credit card.
- You must fax the requestor's official state-issued photo ID. The New York City records office will only process requests followed by a faxed copy of the requestor's government issued photo ID. Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records.
- Photo ID address, billing address, and shipping address must all be the same. THE NEW YORK CITY RECORDS OFFICE WILL ONLY SHIP THE CERTIFICATE TO THE CORRECT BILLING ADDRESS OF THE CREDIT CARD USED.
- IF YOU DO NOT CURRENTLY HAVE A COPY OF A VALID I.D., YOU MAY CALL CUSTOMER SERVICE FOR ASSISTANCE AT 1-888-803-1118
- A signature under the copied ID is also required. Unsigned applications will not be processed.

Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 1-888-803-1119.
Product Information
The following prices are charged by the state of New York City records for the actual certificate and the shipping. The $19.95 fee that is charged by Express Birth Records will be a separate charge from the state's charge.
