Requirements
The following requirements and reminders apply for the state of Ohio. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- You must be an immediate family member or demonstrate a legal right to the record in order to obtain a copy of the record.
- No physical ID required.
- THE COUNTY OFFICES ARE HOSPITAL SPECIFIC; IF YOU DO NOT ENTER THE NAME OF THE HOSPITAL, OR IF THE HOSPITAL YOU ENTER IS NOT IN THAT COUNTY, THE ORDER MUST BE PLACED THROUGH THE STATE RECORDS OFFICE IN COLUMBUS.
- All OHIO OFFICES WILL SHIP EXPRESS COURIER ONLY TO HAWAII
Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 1-888-803-1119.
