Requirements
The following requirements and reminders apply for the state of South Carolina. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- Eligible requestors include: self, or parent, must provide a copy of a valid I.D.
(A married female ordering a certificate for herself, who has changed her last name due to marriage, must provide a copy of a marriage license as well as her I.D.)
An adult child must have legal custody or control over of the certificate holder. You must provide a certified copy of the court order establishing Power of Attorney. You must also provide a copy of your valid I.D.
A spouse must provide a notarized letter of permission from your spouse (the certificate holder) and a copy of their and your valid I.D.'s

Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 877-684-5852 .
