Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 1-888-803-1119.
Product Information
The following prices are charged by the state of Vermont for the actual certificate and the shipping. The $19.95 fee that is charged by Express Birth Records will be a separate charge from the state's charge.
